Fleshrender is a Druid-exclusive Unique One-Handed Mace in Diablo 4 that turns every Defensive Skill cast into an area damage burst against Poisoned enemies. Each time you use a Defensive Skill — Debilitating Roar, Earthen Bulwark, Cyclone Armor, or any other skill in the Defensive category — it deals 300–600 damage to all nearby enemies that are currently Poisoned. That damage increases by 50% for every 100 Willpower you have, meaning the more Willpower you stack on gear and paragon boards, the harder each Defensive Skill cast hits the surrounding Poisoned targets. The +4–5 to Backlash passive and 8–12.5% Defensive Cooldown Reduction mean you cast Defensive Skills more often and each cast hits harder through the Backlash damage scaling. Fleshrender works on a principle that any experienced Druid player will immediately appreciate — defensive actions should not be wasted turns. In most builds, casting Debilitating Roar or Earthen Bulwark is purely a survivability choice with no offensive return. You take a moment away from dealing damage to protect yourself or reduce incoming hits. Fleshrender eliminates this tradeoff entirely by attaching an area damage burst to every Defensive Skill cast, meaning every moment spent staying alive is simultaneously a moment spent dealing damage to every Poisoned enemy nearby.
The Willpower scaling is what makes Fleshrender particularly well-suited to Druid. Willpower is the Druid’s primary offensive stat — it increases all skills’ damage and is the stat that Druid builds naturally prioritise across every gear slot and paragon board investment. Fleshrender’s burst damage scales with this same stat rather than requiring any separate investment, meaning a Druid that is already optimised for general damage output is also automatically optimised for the maximum Fleshrender proc damage without changing a single affix priority.
The Defensive Cooldown Reduction affix on the weapon helps maintain the burst frequency. Shorter cooldowns on Debilitating Roar and Earthen Bulwark mean those skills come back faster, which means the Fleshrender damage burst fires more often throughout any extended fight. Combined with the +4–5 to Backlash — which adds its own area damage to each Defensive Skill cast on top of the unique effect — Fleshrender makes every single Defensive Skill use in a Poison build a genuinely impactful offensive moment rather than a defensive pause.
