Hands of the Worldbreaker is the item that completes the Apocalypse Warlock’s core identity. Without it, Apocalypse is a powerful Ultimate skill with a substantial cooldown and a straightforward explosion — impactful but not build-defining. With it, Apocalypse becomes a kill-accumulation nuke whose damage ceiling scales with how aggressively the Warlock has cleared enemies within their Sigil before detonating — and simultaneously gains the Sigil tag that feeds the Rite of the Nameless stacking multiplier on every cast.
The 340–400% damage range is the most important roll on the item beyond Greater Affixes — closer to 400% at high item power means the kill-scaled burst delivers nearly 5x base Apocalypse damage at full kill accumulation before the Rite of the Nameless multiplier, Fleshwrit Carapace Sigil damage bonus, and other gear multipliers compound on top. The gamems.com guide notes this specifically: a higher item power drop pushes the percentage closer to the 400% ceiling, making the item power of the specific drop meaningful.
The Sigil tag interaction with Fleshwrit Carapace creates a natural pairing. Fleshwrit Carapace grants Sigil Skills 70–90% increased damage and generates 1 kill per second on active Sigils while saving unused kills on expiry. With Apocalypse gaining the Sigil Skill tag from Hands of the Worldbreaker, Apocalypse benefits from Fleshwrit Carapace’s 70–90% damage bonus simultaneously with the 340–400% kill-scaled bonus — stacking both Sigil-specific multipliers on the same cast. The passive 1 kill per second from active Sigils then passively builds the kill count that determines how much of the 340–400% multiplier Apocalypse receives.
