Hellhammer is a Unique Two-Handed Mace built exclusively for the Barbarian and the weapon of choice for Upheaval builds in Lord of Hatred. Its Unique Effect gives Upheaval the Iron Burst Variant, which transforms the skill from its standard form into a Physical-based attack that applies Bleeding instead of the previous burning ground effect. On top of that transformation, Upheaval deals 60–75% increased damage. The Iron Burst Variant opens up Bleed-related multipliers, aspects, and Talisman synergies for the Upheaval build that the old fire version could not access, making Hellhammer the item that defines which direction the entire Upheaval Barbarian build goes in Lord of Hatred. Because affixes are randomized in Lord of Hatred, secondary stats will vary by roll, but the Unique Effect is always fixed. Hellhammer drops at any difficulty and can be found from Nightmare Dungeons, World Events, Helltide chests, and general open-world loot, with increased drop rates at higher Torment tiers.

Hellhammer Mechanics Explained
The Iron Burst Variant changes Upheaval’s damage type and application entirely. Where standard Upheaval deals Physical damage in a forward-facing slam, the Iron Burst Variant causes Upheaval to deal Physical damage with a Bleeding component, turning it from a burst-only skill into one that leaves damage over time on the target. This shift matters because the Barbarian’s Bleed toolkit — including aspects like the Bloodbath Aspect, Rage of Harrogath’s cooldown reduction on Bleed application, and Talisman charms like the Rend and Rupture interactions — all become relevant to an Upheaval build the moment it applies Bleeding through Hellhammer.
The 60–75% increased damage bonus applies on top of this reworked variant, meaning every Upheaval cast is both dealing more upfront Physical damage and applying a stronger Bleed. The Icy Veins Upheaval guide specifically notes that the Iron Burst variation creates a distinct pure Bleed version of the build that gives players more ways to stack Bleed-related bonuses than the old fire-based version ever allowed, since fire damage had fewer multiplier sources available for the Barbarian in Lord of Hatred.
The roll on the damage multiplier itself is meaningful. The difference between 60% and 75% increased damage per cast accumulates significantly over a sustained fight, particularly since Upheaval is typically cast repeatedly in endgame content rather than used as a single cooldown.
