Twin Strikes is a Unique Gloves exclusive to the Barbarian and the dedicated item for any build centred around Double Swing. The gloves work in two stages that both reward continuous use of the skill. The initial sweep of Double Swing deals 45–60% increased damage and applies Vulnerable to enemies hit — giving your follow-up swings and any other skills in your rotation a damage bonus against those targets immediately. Every subsequent swing then deals 90–120% increased damage and grants Berserking for 3 seconds, keeping you in Berserking uptime as long as you keep swinging. Because affixes are randomized in Lord of Hatred, secondary stats will vary by roll, but the Unique Effect is always fixed. Twin Strikes drops from Grigoire and can also be found through Nightmare Dungeons, World Events, and general open-world loot at Torment difficulty.

Twin Strikes Mechanics Explained
Twin Strikes restructures Double Swing into a two-stage damage tool where both the opening hit and the follow-up hits serve distinct purposes. The first sweep of each Double Swing applies Vulnerable to whatever it hits while dealing 45–60% increased damage. Vulnerable is one of the strongest multipliers in the game, and applying it on the very first hit of a skill rather than through a separate debuff step means your follow-up swings and any other damage source in your rotation immediately benefit from the Vulnerable multiplier on those enemies.
The subsequent swings — every swing after the first in a continuous Double Swing rotation — deal 90–120% increased damage and grant 3 seconds of Berserking each. This is where sustained Berserking uptime comes from with Twin Strikes equipped. As long as you keep using Double Swing, Berserking refreshes continuously from the subsequent swing damage bonus, keeping the offensive buff active without needing separate Berserk-upkeep mechanics or cooldown management. The 3-second duration means a Double Swing cast every 2–3 seconds is enough to maintain permanent Berserking in combat.
The damage values on the gloves also scale in a meaningful way. The roll range between 45% and 60% on the initial sweep and between 90% and 120% on subsequent swings represents a significant difference at the high end — a maximum roll produces substantially more damage per swing than a minimum roll, making the Maximum Unique Power Roll option genuinely impactful for Double Swing builds.
